The Care of the Older Person and Community Transition (CO-ACT) Program supports older adults in their transition from hospital care to community living. Designed for those who have completed their hospital stay but are awaiting placement in Commonwealth Home or Residential Aged Care, CO-ACT offers a temporary, welcoming environment with professional, experienced staff to support day-to-day needs.
A home-like, supportive space: In CO-ACT, consumers enjoy a more homelike environment than a hospital, with options for shared or private rooms (based on clinical needs), beautiful gardens, shared dining areas, activity rooms, and 24/7 nursing care.
This supportive setting fosters dignity and engagement as we help you or your loved one transition to long-term Commonwealth aged care.
Specialised, safe design: Our site includes high-contrast signage, adjustable furniture, and handrails, ensuring accessibility and safety for older adults, including those living with dementia.
Comprehensive, consumer-centred care: Our team of nurses, allied health staff, doctors and other support staff prioritise the needs of each individual, their family, and carers. This approach offers a broad range of services focused on respect, connection, and personalised care as we assist consumers in finding a suitable aged care placement in the community.

The Geriatric Evaluation and Management (GEM) Unit provides specialized, high-quality care for older South Australians facing age-related health challenges such as dementia.
In this unit, consumers are cared for by a multidisciplinary team under the guidance of a specialist geriatrician with a focus on addressing the complex needs of aging.
The primary goal of GEM is to assess individual health concerns and establish a gentle rehabilitation program aimed at reducing disability and the potential need for residential care.
GEM services often support consumers who have experienced falls, are living with dementia, or are managing a variety of other medical conditions that impact daily living and self-care.

The Transition ward provides patient-centred care for individuals preparing to return to their home and community. It is a comfortable, supportive space for patients transitioning out of hospital into the community.
Patients will often move to the Transition ward while their discharge planning is being finalised in collaboration with relevant community providers (for example the National Disability Insurance Scheme).
Patients in the Transition ward receive maintenance therapy, as needed, to help them retain or improve function and independence while awaiting discharge to their next place.

As dementia progresses, many people experience behaviours and symptoms that go beyond memory loss.
These are known as behaviours and psychological symptoms of dementia (BPSD), and they vary widely from person to person. Common examples include agitation, aggression, wandering, restlessness, anxiety, depression, and mood swings.
The team in this unit, understand how to care for patients demonstrating these behaviours and use a patient centred approach to support a positive quality of life.
