The Transition Ward provides patient-centred care for individuals preparing to return to their home and community.
It is a comfortable, supportive space for patients transitioning out of hospital into the community.
Patients will often move to the Transition ward while their discharge planning is being finalised in collaboration with relevant community providers (for example the National Disability Insurance Scheme).
Patients in the Transition ward receive maintenance therapy, as needed, to help them retain or improve function and independence while awaiting discharge to their next place.
